Not sure if the difficulty dipswitch actually matters in this game, but it was indeed set to Hardest (haven't noticed the difference, might be the case of enemies dealing more damage, not sure).
No save states, no cheats, no turbo.

I learned this in one morning actually. I've heard people say this is brutally difficult, others say it's an easy game. The truth is - it's both. If you keep approaching enemies and just fight on your instincts - it's gonna destroy you in certain areas. If you know all the strats, it becomes pretty managable. The problem is the game is so janky that a lot of strats seem inconsistent at first, but with practice you can get them to work most of the time.
There's multiple viable strats for most tricky rooms.
Despite being so clunky and super old school, it's a very fun game, probably my 2nd favourite beat 'em up that predates Final Fight (Golden Axe being Nr 1). Kinda gives off a Vigilante vibe, only with a Y axis.
I've learned most of the strats from 1CC Marathon (player: Scoop, commentary by Zuq).
Figured a few things on my own, like the sweep / check for dizzy option select. And when the enemy is dizzy, it's Special time - Jackie's special finisher is much cooler than Mike's, but overall there seems to be way more differences between the two playable characters than meets the eye.
