Super summarized version: A lot of player requests are counterposed to each other and I don't envy GGG's position in working out how to balance these requests. Adding new 'Uber' versions of content risks turning existing good loot drops into obsolete rubbish.

GGG should go back and study WHY some uniques worked so well as chase rewards - Watcher's Eye and Voices-1/Voices-3 in particular - and use systems like these for aspirational content. 'The same item but slightly better' makes a good chase reward, e.g. if an Uber Esh's Breachstone is added, have it drop a slightly better Hand of Wisdom and Action with the 8-12% stat roll range improved to 8-14%.

Also they should take the defensive mechanic overhaul very seriously, as Fortify in its current stats holds a lot of the game together. IMO player life pools should increase significantly alongside significant nerfs to '%lifegain on block', '%lifegain on kill' mechanics and to the block and dodge caps. As an overall package, the goal should be the same number of player deaths as now, but for the time taken to drop from full life to 0 to be in the 750-1250ms range. (Currently, it's typically in the 200-400ms range in most of my deaths).
