After you beat the game, you have a second quest to complete. New enemies, faster enemies, level variants, etc. Basically, Second Quests on NES games are entirely different games or "sequels" if you will.

It is hard to say if Second Quest is any more difficult than Super Mario Bros 2: The Lost Levels or not.
