Here's how it actually works:
The heart always begins with a debuff. Then, every 3 turns, starting from turn 4, gains a Buff. This Buff includes setting it's own strength to zero, if it's strength is less than zero.
"Strength down", like Disarm, is a single effect, appearing as one icon under the health bar. "Strength down UNTIL END OF TURN" is two effects. The first effect is the strength down, just like disarm, appearing as one icon under the health bar. The second effect, and the second icon, is to gain strength at the end of this turn.
Therefore, if you play "Dark Shackles+" on the heart on turn 4, (which typically would NEVER happen because the heart NEVER attacks on turn 4), the heart gains two icons underneath it's health bar: "-15 strength", and "+15 strength at the end of this turn". At the end of turn 4, the heart sets it's own strength to zero, removing the "+15 strength" icon and its effect. The "+15 strength at the end of this turn" however, remains... and takes effect... at the end of the turn.
