These DAI codex entries are found in various places in the game. Chapters One through Nine are found in the base game, and Chapters 10-??? are found in Trespasser. Pretty much every named character in Varric's most famous novel seems to be based on someone in DA2. Can you figure out who is who? The last entry (Chapter ???) is found in the Vir Dirthara, a library of memories that looks like books. Varric claims that he never wrote this chapter, and it features a character that is suspiciously like who you left in the fade during 'Here Lies the Abyss' main quest. It's unknown how this came to be, but all entries end with a sense of serenity, perhaps a message from those left behind that they are at peace with their sacrifice.

And yes, the codex entries go from spelling out the numbers (...Seven, Eight, Nine) to using the numerals (10, 11, 12...). This and all spelling errors from the original codex entries were kept in the on-screen text.
