Cyborg (http://www.mobygames.com/game/apple2/cyborg) was a text adventure game published by Sentient software in 1981 for the Apple ][. The player controls a cyborg who has lost all its memory and woke up in an alien forest.

I believe it was the first game created by Michael Berlyn (http://www.infocom-if.org/authors/berlyn.html), who later joined Infocom to create games such as Infidel and Cutthroats.
