One of my all-time favourite video games is F.E.A.R., but I've never been able to complete it in the toughest setting since it's so scary.
You are being hunted by an entity—in this case, a ghostly child—which is why this game is so disturbing. If you can escape the child, you'll be safe for a while until it reappears later in the game (and again in your dreams). The child attacks you from behind and tries to strangle you with its invisible hands.
