I have only gotten this glitch to happen with the above conditions. It also triggers during loop 1 and 2. I have not fully figured out what actually triggers the event other than you must leave the room going forward, and then going back. Crossing to the area with the gears seems to trigger it but the first couple of times, I managed to trigger it while in the middle of the room. I was unable to trigger it with 1 player (Legend difficulty.)
