In December 1998, three months after escaping from Raccoon City as shown in Resident Evil 2, Claire Redfield embarks on a mission to find her brother, Chris Redfield, who went missing. She conducts a raid on an Umbrella Corporation facility in Paris but is caught by the security forces and imprisoned on Rockfort Island, owned by Umbrella in the Southern Ocean. During her captivity, a T-virus outbreak occurs on the island, leading to chaos.

Claire is unexpectedly released by her captor and teams up with Steve Burnside, another prisoner, to survive and escape the island. As they explore the place, they encounter Alfred Ashford, the unstable commander of the island, who switches between his own personality and that of his twin sister, Alexia. Eventually, Claire and Steve secure a seaplane and attempt to flee, but Alfred pursues them and redirects the plane towards an Umbrella facility in Antarctica, which is also facing a T-virus outbreak.

Arriving in Antarctica, Claire and Steve find the facility overrun with zombies and monsters. They manage to confront and fatally wound Alfred before he frees his sister Alexia, who had been in cryogenic sleep after injecting herself with the T-Veronica virus, an experimental virus developed by the Ashford family.

Meanwhile, Chris Redfield learns of Claire's situation from a message sent by Leon S. Kennedy and travels to Rockfort Island to find her. After discovering she's already left, Chris heads to Antarctica to track her down. There, he encounters Albert Wesker, an independent agent seeking the T-Veronica virus sample. Chris eventually reunites with Claire and helps her search for Steve.

In Antarctica, Steve has been experimented on and infected with the T-Veronica virus, causing him to mutate. He briefly loses control and attacks Claire but regains his senses and turns on Alexia. In the ensuing battle, Alexia mortally wounds Steve, and before he dies, he confesses his love for Claire. Chris and Wesker confront Alexia, leading to a conflict in which Wesker manages to escape with Steve's corpse for further experimentation. Chris successfully defeats Alexia, and they manage to escape the facility before it self-destructs. -----------------------------------------------------------------
