The map is seemingly extracted from one of the Serious Sam Alpha 2000 levels. However it seems to only import the last part of the level and didn't include the really fun slides :(

This is a faithful recreation of the level with the correct enemy spawns (minus the mammuts which are replaced with ice bulls)
