Follow these simple steps in order for you to get the In Water Ending. In Water Ending easy guide.

Firstly you need to inspect Angela's knife about 10 times just before the dual pyramid head boss fight.

Then let the pyramid heads inflict damage on you and make sure to keep your health at critical level (literally one or two blows from death). To check how low your health is go to your inventory if everything is red than that means that your health is critical.

Then after the dual pyramid head fight DO NOT HEAL.

After the fight make sure to enter the rust colored door. That's the door in which you placed the rust-colored egg.

Make sure to listen to Mary's entire monologue.

After that in the final boss fight, also make sure to maintain your health at critical level, again do not heal, and that will grant you the In Water ending.
