South Park: The Fractured but Whole is a role-playing video game that is viewed from a 2.5D, third-person perspective. The player controls the New Kid as he explores the fictional Colorado town of South Park, which can be freely moved around.[5] Certain areas of the town are inaccessible until the player gains certain allies that can use the New Kid's farting power to overcome the obstacle: Captain Diabetes can be sent into a rage that allows him to move heavy objects; the Human Kite can fly the New Kid to otherwise unreachable heights; Toolshed[citation needed] can clear paths that are blocked by lava (red Lego bricks) that cause the player to burst into flames on their touch. A fast-travel system allows the character Fastpass to transport the player between unlocked travel points.

The game allows the player to select from ten different superhero classes: the nimble Speedster, the short-range, high damage Brutalist, the long range, moderate damage Blaster, the weather-based Elementalist, the offense-attracting Cyborg, the long-range, the status effecting Psychic, the stealthy Assassin, the gadget-using Gadgeteer, the healing Plantmancer, and the high-damage, swift Martial Artist. Powers from different classes can be used together, for example allowing the player to use the Blaster's fire blasts, the Assassin's knives, and the Martial Artist's kicks. The New Kid can eventually eventually have access to four classes at once. Not all classes are available at first, but instead, players, after choosing between Brutalist, Blaster, or Speedster, will be able to choose three more classes and select what abilities they use in combat.

Unlike The Stick of Truth, the game allows the player to choose their gender of their character which will affect how other characters react to the player. The Fractured but Whole also features race customization, which is labeled as a difficulty level slider. The slider doesn't affect combat difficulty, but rather how the player talks to other characters and the amount of money the player receives. The player is able to modify the New Kid's appearance, superpowers, gender, sexuality, race, and ethnicity at any time in the game. Costume items can be collected and mixed together freely and recolored, and have no influence on the New Kid's abilities.

The Fractured But Whole retains the turn-based combat of The Stick of Truth but instead of taking place on a static plane, battles now take place on a variable-size grid on which the player and enemies can move freely to create strategic advantages. The grid can contain environmental hazards such as exploding chemical barrels, and red Lego. Attacks are selected from a menu, and can be augmented with timed button presses,[15] while timed buttons used to defend from attacks can recover some health after being hit, and builds up the super meter. Attacks offer different effects, such as abilities with "knockback" damage which can be used to forcibly move an opponent around the battlefield, and can inflict status effects such as "grossed out" (poisoned), freeze, burn, and bleed. Each attack affects different areas of the grid, requiring strategic positioning on the grid to impact enemies and avoid attacks.

Throughout the game, the player can select up to three allies from 12 different characters to accompany them in battle, each possessing three powers and a powerful ultimate attack. The non-player character, PC Principal, can teach the player the ability to recognize microaggressions, which allows the player a free attack in battle against an enemy that says a demeaning term for a demographic. Additionally, the New Kid possesses the ability to manipulate time with their farts, which can be used in battle to skip an opponents turn, temporarily pause time to inflict damage out of turn, or summon a past version of the New Kid to join the battle. Certain characters, such as Jimbo and Ned, Classi, Gerald Broflovksi, and Moses, can be summoned during battle; the former three delivering powerful attacks, while Moses heals all allies.

There are three difficulty levels, the highest labelled "Mastermind". A crafting system allows the player to use recovered resources such as duct tape, glue, sports drink bottles and taco-related products to craft items such as healing burritos and quesadillas. Some items can also be bought from vendors.

The player is encouraged to explore the wider game world to find Yaoi art depicting intimate moments between the characters Tweek and Craig, or take selfies with the town residents to increase their followers on the player character's Instagram-like Coonstagram social media feed. It is necessary to amass a number of followers to complete certain story missions.
