So, I came across this glitch during one of my attempts. As I start 3-3, a Mission Failed message appears... And the game soft-locks. For context, I have no idea this message existed until now (could be a Retrieve Item mission that fails when the item falls down a hole). I don't know what causes this glitch though. The mutators I used are the same in my previous videos, and it's the same character, too.

Also, stage 3-3 in this run has the Killer Robot disaster. Not sure if that's related or not. Unfortunately, I didn't think of pausing the game before the soft-lock happened to see the seed used here, so unless the same seed is generated, there's no guarantee to reproduce the glitch again.
