To unlock Dr. Mario simply play 50 matches. Dr. Mario will then challenge you, win and you'll unlock him.

Don't worry if you lose, you'll get a rematch after your next match played. A quick way to get through matches is to set the game to Stock mode with only 1 life and walk off the stage as soon as the match starts. You don't have to win matches to get challenged by the secret characters but simply play the specific amount win or lose
