Released in October 2024, it serves as a narrative bridge, connecting the story between the first game, The Coma and its sequel The Coma 2: Vicious Sisters.
Players once again take on the role of Youngho Choi, the protagonist from the original The Coma. Having seemingly escaped the terrifying "mirror dimension" known as The Coma, Youngho finds himself back at Sehwa High School, only to discover that something is still amiss. His new adventure drags him back into this warped reality as he attempts to uncover what truly happened between the events of The Coma 1 and The Coma 2.
Youngho's primary objective is to collect pages of Yaesol's notes, which lead him on a desperate search for a powerful relic sword. This weapon is crucial for combating the relentless antagonist, the Vicious Sister, who continues her pursuit. The storyline becomes more complex with the introduction of an unwanted invader into the mirror dimension and a new character with ties to Yaesol who appears to aid Youngho.
Players will explore twisted and nightmarish versions of familiar and new locations within the Sehwa District, including the school itself, the mysterious Dokaebi Market, a vast underground facility beneath Dokaebi Market, caves, and a hospital. Throughout his journey, Youngho must evade the relentless psychopaths, solve puzzles, scavenge for resources, and discover chilling clues to unravel the dark secrets binding these interconnected narratives, all while battling for survival against terrifying odds.
