These are, in my opinion, the 3 hardest spells in Touhou 14. Raiko's last 2 are pretty rough too, but they're at least enjoyable for me. Well, maybe not Blue Lady Show, but that one I think I can get down reliably. I've captured every extra and lunatic spell by now, so I know which ones I found hardest.

Fang Sign "Canine Teeth Under the Moon" - Kagerou Imaizumi, Stage 3, midboss spell.
yeah I don't know what ZUN was thinking with this one. The waves come way too fast, if anyone has a reliable method for capturing this that'd be great, but I'm stumped. Capped on the 3rd try because of some good luck.

Echo Sign "Echo Chamber" - Yatsuhashi Tsukumo, Stage 4B, 2nd boss spell.
I just can't follow this pattern. It's one of those things that's beyond my current skill level, except by a fluke. Captured on the 24th attempt with a lot of lucky dodges.

Koto Music "Social Upheaval Requiem" - Yatsuhashi Tsukumo, Stage 4B, 3rd boss spell.
Nearly impossible. I fluked into this captured on attempt 37. Never going for this in an actual run...I dread going for a perfect of Yatsuhashi.
