The C5 Galaxy AirCrash Happened In Dover AFB. The "Reverse Unlock" alerted the crew of reverse sleeves open on engine #2 during initial climb. The proper action is to shut down the faulty engine. Captain made the decision to return to origin airport immediately without any non standard QRH procedures, proper cockpit preparation and proper crew briefing. Some of the crew members were surprised finding them on final approach for RW32. The sequence of pilot mistakes with reducind the wrong engine and extending the flaps to 90% instead of 40%, bad teamwork and CRM led to that air crash. Luckily everyone survived that crash, that is why we have that accident inour review.
