Just a quick video showing how to easily get the "And Clear" achievement. Play on Story difficulty with the tutorial disabled. For your starting squad we want the highest damage possible, so picking the three members with shotguns is the best option (Godmother, Axiom, Claymore) and then one with an Assault Rifle. I picked Verge. Pick Gray Phoenix for first investigation and buy 2 tracer rounds. Give them to who you want to go 2nd and 3rd during the first breach. The first mission breach has 4 enemies, 2 legionnaires (5hp) and 2 Adders (3hp). Check what breach options you have. I saved Seal the Deal (+3 damage) for one of the legionnaires, and tracer rounds for the 2 Adders. The first one through shoots the closest legionnaire. If you only do 4 damage, restart and try again.
