Avatar of Khaine is a fragment of an Eldar god Khaine.
Around the 31st millennium the Eldar race birthed a god of chaos called Slaanesh due to extreme debauchery by them. In that instant most of the Eldar and its other pantheon of gods was consumed by Slaanesh and Khaine fought it to a stalemate. However Khorn another god of chaos believing Khaine belonged to him because he is god of war, fought Khaine also and in that instant, Khaine was shattered into many fragment. Each fragment landed on the craftworld that escaped the birth of Slaanesh and so only in extreme dire situation will the Eldar use the fragment of Khaine in war. Because Khaine makes Eldar go berserk and in order to change into an Avatar of Khaine, they have to sacrifice a Farseer ( I might be wrong).
Remember its because of the birth of Slaanesh that the Imperium was able to launch the Great Crusade.
