The question mark block was not fully on the screen so the block did not function.

I think this is common in NES games where it is very easy for something to not happen if it is not completely on screen. Like a Koopa shell disappearing into thin air right after being kicked to the left of the screen. Somehow though, Koopa shells are still in existence when going to the right. They would surprise you in Mario 1 where they would bounce off a wall and come flying right back at you.
