Holstering your weapon in the game can cause cancelation of animation sequences like reloading. If done just right, you can use the alt. fire of the Sawed Off Shotgun much faster and this creates an imbalance to those either unaware or have a hard time doing it right since you have to use both the holster key and the weapon key to switch back.

In Bloodbath/Teams, this can create a balance issue but remains useful in Singleplayer and Cooperative game modes. It also appears to be useful in speedrunning as well.

Also yes, I had custom sounds installed on my NotBlood copy, you can do that easily by dropping .RAW files to the root folder.
