The playstyle is to group a bunch of monsters together, and stay alive while you punch one of them in the face a zillion times. When it dies, thanks to the Blooadsoaked Garb set, it will explode and deal tons of damage to everything nearby.

Attack speed is at 303%, to hit the 300%+ breakpoint.

CDR is basically exactly enough to have 100% uptime on the attack speed buffs.

Health on hit is enough for the level shown here.

Reach is important for pull range.

If you are able to survive well without Frostcall, then Earthen Strength (physical ultimate) offers more because you don't have to stretch your skill points through a bunch of unused cold skills.
