Reminder: First thing when you start the game as level 1, grab the weapon off of the follower who is a similar class to you. This will give you a damage boost for the first few minutes and only takes a couple seconds.

We played almost the whole time on "Hard" difficulty except for around levels 40-50 where we bumped it up because we were killing extremely fast but we went back down to "Hard" pretty quickly.

This was our first try and didn't even "optimize" our builds or anything, and this is on hardcore mode where we had to play extra careful so this should be something most people can repeat.
