This finished uploading before my playthrough of the main game, so I'll say this here: Doom Eternal is possibly the best FPS I've ever played, and the DLC was also quite fun as it kicks the difficulty up a notch. I enjoyed how new enemies force you to use some of the weapon mods you would have skipped over in favor of their more powerful versions. I'd heard that there had been significant nerfs to this DLC, and was definitely able to notice at least one of them (turrets staying out longer) but didn't play pre-nerf so don't know the enemy composition changes closely -- it still feels plenty challenging.

This DLC is a lot tougher than the base game, but if you skip over optional encounters and thoroughly practice I found it easier to deathless just due to the short length. I practiced for 6 hours and got it on my first intentional run (I accidentally started one other run on UN when I meant to just practice on nightmare, and only realized when I died).
