There are 4 hidden gems in Exophobia, one on each floor, which are required in order to get the "good" ending. After you have collected all four gems, go and complete the eleveator fight at the end of the game. Once you enter the room with all the remaining humans, put the gems into the device in the back wall of the room and then you will have one final boss fight against the Alien King. Upon defeating him, you'll get the true ending and unlock the "Alien King" achievement.
