Level 25 "The Temple Of Darkness" of "Plutonia Experiment" with "Hunter's Moon" mod... and the third temple I did ever visit (with Temple Of Retribution being the first and Temple Of The Portals being the second).

According to the description, this level is all about ambushes. And I'd say yes, it has ambushes... which are quite easy to cope with, if to mix caution and aggression the right way.
Btw, the second one was kinda disturbing, not because of its difficulty, but because of those "squeaky-voices" (yes, that's how I do call Berzerk Hunters)
