You must be in Chopin mode (hardest difficulty). You can do this on any level, but I think that level 1 is the easiest. Get a score above 40,000. The trick is to hit as many red note combos as you can (10 red notes = 2000 points). Make sure to hit the gold notes when you get a combo, too, for ~1000 points each. Red note on a black key is in the middle and a red note on a white key is also in the middle; use the very end of the black keys adjacent to the white keys for reference of when to press for red notes. Practice practice practice, easy enough.
