Upon awakening to the chaos of more Fire Devil attacks and the presence of a nearby helicopter, Denji experiences a hallucination of Nayuta lying on his chest before being abruptly brought back to reality by Yoshida. Confused and concerned, Denji inquires about Nayuta's whereabouts, only to receive the same lack of information from Yoshida. Frustrated, Denji attempts to rise from his bed, only to be reminded that he hasn't walked in about a week, rendering any movement futile. Yoshida then explains that he had tried to simplify the explanation of the consequences for Denji, but now, having broken the rules, he cannot offer further assistance. As Yoshida exits the room, he informs other members of public safety, including Fumiko, that he has sedated Denji. In a macabre exchange, Fumiko eagerly discusses harvesting Denji's nails and hair, anticipating the government's seizure of his body. When questioned about her true motives, she reveals herself to be a collector rather than a mere fan.
Amidst his turmoil, Denji experiences another vision, this time of Pochita urging him to rescue Nayuta. However, Denji grapples with the realization that he lacks the means to fulfill this mission without his legs.
The scene transitions to Denji on an operating table, with one leg already amputated. Surgeons proceed to remove the next leg, while one reprimands a Public Safety member for smoking. Despite the member's duty to prevent Denji, now Chainsaw Man, from escaping, he is ultimately dismissed from the room. It is revealed that Denji is being held in the Tokyo Detention Centre, a facility known for its maximum security, where no devil has ever managed to escape. The gravity of the situation becomes apparent as the centre unveils a contingency plan to thwart Denji's potential escape, symbolizing the formidable challenge ahead, even for a single individual standing guard outside.
