(BUNP) Just before the UB ending select Snap Neck.
(Like a D6) After selecting Sneak In, you need to select Jumble Hoppers 3 times in a row.
(100111010) After selecting Goodball in the JPE route, click OK on 3 of the error messages.
(Connoissuer) There are 30 Paintings lying around in the museum, click on all of them to unlock this acheivement.
(TL;DR) After selecting Wormhole Rifle in the fail text there is a spot where it says "If you want a free acheivement click here" click on the word here.
