3Agent 47 is inside the Gontranno Asylum in Romania, which is now overrun with SWAT teams sent to eliminate him after his rampage in the previous game.
Injured and disoriented, 47 must escape the facility while evading or eliminating SWAT officers. He starts with minimal equipment but can acquire weapons from downed enemies. As he progresses through the asylum’s corridors and bloodstained halls, he faces heavily armed foes while trying to reach the exit.
Eventually, 47 makes his way outside to a waiting car, where his handler, Diana Burnwood of the ICA, helps him escape. The mission ends with him collapsing in the vehicle, setting up the game's flashback-driven narrative.
