To add a friend in Call of Duty: Modern Warfare 3, you can follow these steps:

Launch the game and enter the main menu screen.
Enter the Social tab. By pressing the R3, right thumbstick in.
Click on Add Friends.
Enter your friend’s Activision ID with the numbers.
Click on Send Request next to it.

Your friend will be added to your friend list as soon as they accept your friend request. Press options and go to notifications and you can accept a pending friend request.
