There are 2 glitches here.
First one is the craft-select glitch. This glitch enables you to create high-level mods without the requirements needed.
On consoles, you can make a mod directly beneath the one you can already create. Note that its "create" not "attach".
On PC you can make any mod on the same page by using the mouse cursor, making it even more broken.

The second one is the mod-swap glitch. This glitch lets you put almost any mod on any weapon.

Part 1. Bat + Knife/Ripper
This glitch requires the Nuka Cola DLC.
By making a strong baseball bat mod using the craft-select glitch, and then putting it on a fast weapon such as the combat knife or the ripper, you can make a weapon that does insane DPS on level 1.
Part 2. MIRV + Minigun
This glitch requires the Far Habor DLC.
Since you need to "create" a mod the Fat Man cannot be craft-selected normally. However by using a unique Fat Man called "The Striker" you can actually craft the standard launcher and to an extent, the MIRV mod. Slap this on the minigun and the game is broken.
p.s. The Striker is a unique weapon, so you'll need another Fat Man to do this.
