If you are following the Master Necromancer challenges you'll eventually get to the Terror In The Blood Achievement. This video shows you how to unlock this achievement. It was a hard achievement, no so much that Diablo was hard to beat but more so of having to cast a life consuming skill while under 10% life.

First you will want to use the Act IV The Silver Spire Level 2 Waypoint then make your way to Diablo. You will want to make sure you're not doing any extra thorns or auto damage to Diablo so he does not die before you can take enough damage. What I had to do was change my armor as well to something that would allow me to take more damage but not so much damage that I die quickly. I was using the Grace of Inarius Set, I then waited until the 3rd time Diablo respawns to have him drop my life under 10%. I then casted Blood Rush to teleport away from Diablo and fired bone spears to kill him. Timing is the biggest key here, get it right and you'll unlock the achievement.

Here are all the achievements you will need to unlock to get the wings:

➡ The Silence When You Speak to Me: Unlocked during the campaign, make sure to listen to all of Master Necromancer Ordan's conversations, go back after killing Malthael and there are a few more things he says.

➡ Serve the Balance: reach level 70 with the Necromancer.

➡ Bringer of Order: kill Malthael in Campaign Mode

➡ Bone Skewer: hit 40 enemies with Bone Spear(s) in less than 3 seconds.

➡ What Is Dead Can Sometimes Die: kill 1000 Undead enemies with any corpse skills.

➡ Curse Your Enthusiasm: kill 5000 enemies affected by any Curse.

➡ Terror in the Blood: kill Diablo after casting any Life-consuming skill against him while below 10% Life.

➡ Self Controlled: let Simulacrum kill a Shadow Clone in Realm of Terror

➡ Game of Bones: use Bone Armor to rip the bones from Skeleton King.

➡ Skeleton Crew: kill 500 Elite enemies with Command Skeletons minions.
