The Scalpel has different stats than the rest of the knifes. Optimally should try to get a Scalpel in your loadout since it's a direct upgrade from your perk knife.

Demo in particular benefits a lot from the Scalpel, since his knife gets a massive +100% bonus, and he finds lots of use out of it if you use RPG+C4+.500 Magnum.

I say optimally if you plan on playing demo spawn as Swat. Drop all your weapons and cash at trader. Then swap to Medic. Then type suicide. Then let your team finish the wave. You'll spawn as medic. During trader swap to Demo. You now have the economy boost from Swat. The Scalpel. And since you died, Zeds are less likey to spawn Aggro'd on you so you are safer.
