So, this surgeon is a civil war surgeon. This is part of why I feel like this is a not!Earth. Because this feels like a New England town, but it's close enough to the battle lines to be taking in battlefield casualties. If it is Earth, I guess the Mason Dixon line does run right up to New Jersey and Philadelphia... so I guess it does make some sense.
