Finishing up the game today (surprisingly quickly) and also trying a bit of Extreme mode as well. Noticeably Extreme is much harder due to the lack of rations in most places. I wasn't aware of that and you also take more damage. I think Hard is a pretty comfortable difficulty for me though.
