The following pattern is based upon a suggestion from youtube user https://www.youtube.com/user/retroshinobi

First, make sure you have at least 20 hearts going into the fight for the second form. If you don't have at least 20 when you get close to the fight then just go back down the stairs which will regenerate the candle sticks. Leave two of the four candles in the battle location until after you throw holy water at Dracula so that you can strike them after to get the double shot. This assumes that you hit the last candle to the far left to get the holy water just before you engage Dracula.

Second, stand on the middle of the 5th and 6th stones from the right. After you strike him for the first time you will go to standing spot and then you will need to simply guess where he will appear. In the video I assume to go to the right towards Dracula. If you get hit at that point then get back to the standing spot and then assume a right movement and then you are on pattern. If you don't get hit then continue with the pattern. Therefore, assume to get hit at least once during the fight so make sure you have enough life to get hit at least once. If he spawns to the far left then assume to continue to the right from the standing spot after striking him, and then switch sides if he is close moving towards where he was. Which works nicely also when he is on the left close to me, so when you assume a left movement here you get that extra time to catch him if he spawns on the far left. Generally do a slight pause on the standing position before you make the assumed move, keeping in mind that the spawn will occur about 2 seconds after Dracula disappears. Basically, other than if he spawns to the far left you will assume to move towards his previous location from the standing spot. It seems to work extremely well.
