It's real and it's possible. I have been wanting to do the run for like a year, I started routing on July 2023 and at first I tried to do it featherless and with dual swords as my main weapon. However there were many problems with this, like the run being extremely long, some bosses being inconsistent and having to manage bad rng, etc. It definitely is possible to do it featherless but its much harder.
I started using feathers and speedruns as reference and the game was much manageable as bosses melted as soon as they had 0 ki (except for humans).

Then on December and January I had my route more or less optimized and started trying, doing some small changes as I went. It took months to get this run, with some breaks in between as I got frustrated sometimes.
I'm very happy to have done this run, as I had doubts if it was too much for me or not worth it, but it was very worth it.

As for the world first status, it may be a bit confusing, let me explain. thelastonexo did a nioh 2 run titled no damage run but it isnt fully no damage as they get damaged by fall damage, burn damage etc but they didnt get any damage from enemies. It's an impressive run for sure, but it is not no damage.
