*Update - Bonus Stage #3 is missing here because I was not aware of its existence until last night (4/21/20). Thanks to SuperViperT302 for bringing this to my attention.

Here is some information regarding the requirements to access all 5 bonus stages:

Bonus Stage 1 - The hundreds digit of your score must be a 3, 5, or 7
Bonus Stage 2 - None
Bonus Stage 3 - None
Bonus Stage 4 - You must first pass through the center of the mountain with a hole
Bonus Stage 5 - All Power-Up bars must not be highlighted

Credit to "Perikles STG" (absolutely phenomenal player) who informed me of the existence of bonus stages in this game. Until roughly a week ago, I was completely ignorant of them. There's 2 or 3 videos on YouTube that show Bonus Stage #1, but that's all I could find. Previously, someone had uploaded all 4 of them but that video and their account have since been removed.

Also credit to the "Secrets" section of the StrategyWiki page for Gradius. It does not actually list any bonus stages for the PC Engine version, but it does for the MSX. I used the four depicted secret entry spots shown to aid me in my search while playing the former.
