In this video (IMG 4930), Ray the Service Dog and I demonstrate how to shop with your dog while pushing your shopping cart down the isle. We're at Home Depot in Glendale, California. This is a perfect place to practice as the isles are nice and wide. Home Depot allows pets inside their store as well, so this how-to video is perfect for both service dogs and regular pet dogs.

I really like the large clip I use on Ray's leash as it allows your hands to be free and can be clipped to the cart. Additionally, a 6 foot leash is perfect to allow distance away from the cart, but close enough to control your dog. I do not recommend an extendable leash. Your dog should have the basic training commands under his control before you attempt something more advanced as this training maneuver.
