The story follows Ethan Winters, a civilian with no prior connection to the Resident Evil series, who is drawn to a derelict plantation in Dulvey, Louisiana after receiving a mysterious message from his wife, Mia, who has been missing for three years. The message urges Ethan to find her at the Baker family estate.
When Ethan arrives, he finds the house in disrepair and filled with grotesque creatures. He eventually locates Mia, but she appears mentally unstable and violently attacks him. Ethan later learns that the Baker family—Jack, Marguerite, Lucas, and Zoe—have all been corrupted by a strange, parasitic organism known as the Mold, which gives them superhuman abilities and causes extreme physical mutations.
Ethan is captured by the Bakers and subjected to their horrifying hospitality, but he escapes with the help of Zoe, the seemingly unaffected daughter of the family. As Ethan tries to survive, he uncovers the truth about the Mold and the origins of the family's condition. The Mold is connected to a bio-organic weapon (B.O.W.) known as Eveline, a young girl artificially created by a shadowy organization. Eveline can control minds and spread the infection, and she is responsible for the downfall of the Baker family, considering them her "family" while keeping them under her control.
Throughout the game, Ethan battles various members of the Baker family, each mutated in terrifying ways by the Mold. Eventually, Ethan discovers that Mia had been working as a caretaker for Eveline, which is why she had been missing for so long. In the final act, Ethan fights a monstrous Eveline and destroys her with the help of an unknown organization, revealed to be Blue Umbrella, a reformed version of the notorious Umbrella Corporation.
The game ends with Ethan being rescued by Chris Redfield, a familiar face from previous Resident Evil games, who is now working for Blue Umbrella.
