From what I can determine to get this achievement (even after reading what people already said to get the achievement), you have to: get three Chapter 10 fates correct with the Captain at least, then proceed to blame everyone's deaths (regardless of correct identity) on the Captain, and return to the boat afterwards.

Unfortunately, there's no payoff for all this... not even a remotely different cutscene from what I've examined. Despite the disappointment, still a great game though!

As the inspector, I'm tasked with figuring out what the heck happened to this large 60-person crew and verify everyone's fates through some magic pocketwatch that a survivor provided me to get the complete picture of this ship's trip and the crew's demise.
