In the dgen.cfg file, I changed these lines to look like this to make my Xbox 360 PC controller fully work because start didn't work, and the B button mapping wasn't where I wanted it:
joy_pad1_b = joystick0-button1
joy_pad1_start = joystick0-button7
Just search for the text before the equals signs to find the lines.
