In the aftermath of the thrilling Sengoku era, Isshin Ashina takes command of the Ashina domain. Amidst the chaos, a nameless orphan is taken in by the enigmatic shinobi, Owl, who dubs the boy Wolf and imparts the secrets of the shinobi arts. Fast forward two decades, and Ashina is teetering on the edge of collapse. The elderly Isshin is ailing, and the relentless forces of the Interior Ministry are closing in. In a desperate bid to save his clan, Isshin's adopted grandson, Genichiro, seeks the immortal Divine Heir Kuro, aiming to harness the Dragon Heritage in his blood for an unstoppable army.
Disapproving of Genichiro's plan, Isshin alerts Wolf, now a seasoned shinobi and Kuro's loyal guardian. A fierce battle ensues, resulting in Wolf losing both Kuro and his left arm. Despite the setbacks, Wolf, being immortal, survives and is rescued by a retired shinobi, the Sculptor, who not only nurses him back to health but also gifts him a remarkable prosthetic arm.
Determined to rescue Kuro, Wolf storms Ashina Castle. Along the way, he can confront his haunting past, delving into a three-year-old incident at the Hirata estate led by his former teacher, Lady Butterfly. Wolf fought valiantly but was betrayed and immortalized by Kuro. In the present, Wolf faces Genichiro once more, emerging victorious and forcing him to retreat.
Kuro urges Wolf to perform the Immortal Severance ritual to end the bloodshed. Reluctantly, Wolf agrees, embarking on a quest to gather the necessary ritual components. Along the way, he crosses paths with Isshin, who bestows upon him a new name: Sekiro, the "one-armed wolf."
As Sekiro collects the ritual components, he faces Owl, the mysterious assailant from the Hirata estate. Owl reveals his own pursuit of the Dragon Heritage, demanding Sekiro's loyalty to Kuro. The choices made in this critical moment lead to different outcomes: a descent into darkness with a "bad" ending or a path of sacrifice and redemption.
After defeating Isshin in a gripping battle, Sekiro faces three possible endings. The standard ending involves the removal of the Dragon Heritage at the cost of Kuro's life. The Purification ending sees Sekiro sacrificing himself to sever immortality, working with Emma to find an alternative. The Dragon's Homecoming ending, achieved by assisting the Divine Child of the Rejuvenating Waters, unfolds a poignant journey to return the Dragon Heritage to its rightful place, offering a captivating conclusion to Sekiro's epic tale.
