This bug is a bit more complicated and difficult to explain, so I will have a commentary video following shortly that can hopefully do a better job.

The bug only occurs if you have the "Mix it Up" skill unlocked and will affect weapons on the second slot in all weapon categories. Basically, if you're holding a weapon that belongs to slot 2 and use your "Weapon Selection Modifier" key to select the same weapon, it will remove the weapon from your right hand and have it in your left hand only. To get out of this bugged state, simply tap your Weapon Selection Modifier key, select a different weapon while still holding down this key, or use your "Toggle Dual Wielding" key.

The bug gets stranger still. If you follow the steps above to get this bug to occur but let go of the Weapon Selection Modifier key, then trying to access any weapon in slot 2 will remove the right handed weapon and keep only the left handed one.

P.S., yes I reused the description from the SS4 vid because I'm lazy and will also have a commentary version to explain it there too since you can theoretically have 3 weapons in a single weapon slot.
