You need to destroy at least two Kali altars to get the ball and chain. If you destroy a third one you will get a permanent ghost.
I got the ball and chain at 4-3 so there isn't much gameplay with it in this run. But there will be in later videos.

If you don't know about this game.
Each time you die, you start back at the beginning "1-1". But the levels are randomly generated so they'll be different every time.
There are ways to unlock shortcuts, but the more you play this game the more you realize that's not the fun way to play it.
There also are BUNCH of secret stuff(characters, levels, items etc.). Didn't show any of that stuff on this playthrough.

Once you get into it... it's hard to stop playing. Every run is so different.

This was the X-Box Live Arcade version of the game.
