Clicking out of the projector view after clicking the card on the table and BEFORE the card exits the projector makes the card fail to exit, so it cannot be picked up to progress.

This is caused by clicking and dragging the bacab plate ring and letting go without setting it correctly, then leaving this screen and returning to it. This will cause the bacab plate ring to become unclickable, and therefore it cannot be aligned properly.

This sprite pops up after solving the hieroglyph puzzle. It seems to be leftover from a time in development when Mateusz Skutnik was originally considering some other type of design for the pedestals in question before going with the Eyes of Horus. If you think about it, the original sprite design's green bubble is probably an indicator of some sort, which would make sense to put on the pedestal with the movable blocks but not where you pick up the cog (because then you'd already be able to see the cog on the same screen, so it would be a bit redundant).
Why did no one notice this before now? My guess is that the Eye of Horus connection is stored in your short-term memory only for as long as you need to solve the puzzle, and then when you see the shiny blue ring with the cog inside, the puzzle is done so your mind immediately wipes that connection away. That or people did notice and just decided not to say anything ;P

The "light-activated door" - The stone doors controlled by the rotating cube can be opened, but will still appear closed if the lamp leading to each of the rooms controlled by a stone door has not yet been lit.
