The Sword of Night and Flame can be found in Caria Manor in extreme northwestern Liurnia of the Lakes. You'll must first defeat Margit, the Fell Omen and Godrick the Grafted to gain access to this larger area above Limgrave. Once you do, make your way to the far western side of Liurnia and north up the coastal road. You could also venture through the main part of Liurnia, passing Raya Lucaria Academy and then west from Sorceror's Isle.
