This game is very simple to play with eye tracking/Mill Mouse for looking around smoothly and Voiceattack for movements and actions. You could have one voice command for toggling forward walk and then turn right or left just by eye gaze. You then need just a few voice commands for performing some of the actions.

-------- The game is a bit demanding but my mid-tier with RTX 3060Ti graphics card did well. Just don't expect to run this at 4K or crazy frame rates - which btw is completely pointless on a 24" monitor and in single player games ----

Personally I use two physical switches for right and left, and when I press both simultaneously I walk forward (this can be achieved with two assistive switches connected to an Adafruit Feather ESP32 programmed to act as this kind of specialty keyboard OR if you wanna do this with your current keyboard setup you just need an Autohotkey script - ask for help in ATMakers Facebook group or your local Makerspace group or reach out to tech University students) . There's always a solution out there specific to your needs. Project Iris is also a very good eye tracking software you can use to create your own setups.
