Das Konzept von The Last of Us entstand nach einem Ausschnitt der BBC-Naturdokumentationsserie Planet Erde, welcher eine Ameise zeigte, die von einem Cordyceps-Pilz infiziert wurde. Hierbei übernimmt der Pilz alle Handlungen des Insekts und produziert Geschwülste auf seinem Kopf. Die Idee, dass der Pilz Menschen befallen könne, war die Initialidee des Spiels. Künstlerische Inspirationen kamen von den Romanen Stadt der Diebe, Ich bin Legende, Kein Land für alte Männer, Die Straße, Unterwegs, der Comicserie The Walking Dead und deren Adaptionen.
Die Computerspielwebsite GamesRadar betonte die Inspiration von den Filmadaptionen I Am Legend und The Road, sowie der Fernsehserie von The Walking Dead und den Filmen 28 Days Later, Children of Men und Blumen des Schreckens.
